BOQ in Ilocos Norte starts issuing COVID-19 vaccination certificates

LAOAG CITY, Ilocos Norte — The Bureau of Quarantine (BOQ) in Ilocos Norte has begun issuing COVID-19 vaccination “yellow cards” to fully vaccinated individuals who are set to travel abroad, the provincial government said Monday, August 23.

The yellow card, which is also known as the International Certificate of Vaccination or Prophylaxis (ICVP), is a document created by the World Health Organization to certify that international travelers, including overseas Filipino workers, have been fully vaccinated against COVID-19.

BOQ said all those who need the cards should make an appointment online and will be required to provide details indicated on their vaccination cards, passport, a valid identification card, and a certification from the local government unit which administered their vaccines.

A documentary fee costing P300 and a convenience fee of P70 will be collected from applicants and the fees may be paid virtually or at the office of BOQ. INQ
